INTRODUCTION
City Council is asked to consider the application from Craig Ballard for a massage center license.
BACKGROUND
Craig Ballard has made application for a massage center license for Salon St. John at 705 East First
Avenue.
The customary background investigation has been conducted by the Police Department. The
investigation did not reveal anything that would constitute a basis for denial of the massage center
license. A building inspection has been done of the premises to insure compliance with the City
Code. The massage center is located in the general commercial districts, as required by the
ordinance.
Mr. Ballard has also applied for a massage therapist license. He is in compliance with the
educational requirements and can be issued a license, administratively, by the city clerk, per the
ordinance.
NOTE: At the regular meeting on December 16,2003, City Council directed staff to prepare an
ordinance repealing the massage center and massage therapist regulations. That ordinance has been
prepared for Council consideration. Because Councilor Joos was going to be absent at the January
6th meeting, the ordinance was not put on that agenda. Because Amy Snyder desires to be present
when this matter is put on the agenda, and because she is unable to attend the January 20th meeting,
the ordinance is not being put on the January 20th agenda. It is anticipated that the proposed
ordinance will be on the February 3rd agenda.
Mr. Ballard has issued gift certificates for a massage and is anxious to begin honoring them. If and
when an ordinance is adopted repealing the current licensing regulations for massage centers and
massage therapists, Council will be asked to refund all fees paid for the 2004 licenses, upon
surrender of the licenses that have been issued.
RECOMMENDED ACTION
Approve the application and grant a massage center license to Craig Ballard dba Salon St. John at
705 East First Avenue.

Enclosed are your Massage Therapist License and Massage Center License for 705 East First
Avenue.
It is my understanding that you are currently enrolled at Touch of Life School of Massage and
have not yet received a diploma or certificate of graduation.
Please note that these licenses are issued pursuant to the City Code, Section 6.41 Subd.3.D.1.c.
This section permits a license to be issued to a student who is currently enrolled in an approved,
accredited, or licensed school and who has successfully completed at least 160 hours of course
work that wi11lead to a degree within one year. You will need to complete your schooling and
provide evidence of a diploma or certificate of graduation in order for your licenses to be
renewed for 2005.
